Your Privacy and Security are Very Important to Us
Your personally identified information will be kept confidential as required by law. Federal Privacy Regulations provide safeguards for privacy, security, and authorized access. Every effort will be taken by Virginia Commonwealth University (VCU) to protect your information. Information that could directly identify participants, like name, date of birth, address, and telephone number, will be removed from study records and will be encrypted and stored separately, accessible only by the study Principal Investigators (Drs. Fuemmeler and Burch) and approved researchers. Information that could directly identify you, like name, date of birth, email address, and telephone number, will be separated from the answers to the survey. Your survey responses will be coded and will be stored separately, accessible to only the study Principal Investigators and authorized research staff. Electronic information will be saved as password-protected, encrypted files and stored on a secure server behind firewalls at the VCU School of Medicine. You will not be identified unless it is required by law, or for your personal health care or safety.
Researchers may have to give your information if the study is audited, or if the information is required by the Food and Drug Administration (FDA). The researchers cannot prevent you or others, for example a member of your family, from sharing information about you or your involvement in this research.
